Preparing for long-term success before and after bariatric surgery

My goal is to provide a supportive, collaborative, and nonjudgmental evaluation experience that helps prepare you for long-term success after bariatric surgery. Most patients describe the evaluation as feeling much more like a conversation than a test.

ThriveWell Psychology provides pre-bariatric surgery psychological evaluations for adults throughout Minnesota. All three appointments can be completed through secure telehealth from a private location within Minnesota. Insurance and self-referrals are accepted.

Why Is a Psychological Evaluation Required?

Psychological, behavioral, and emotional factors can play an important role in long-term outcomes after bariatric surgery. The evaluation helps identify your strengths, support needs, and recommendations that may improve your preparation, adjustment, and long-term success.

Will I Pass the Evaluation?

The purpose of the evaluation is not to decide whether you are a “good” or “bad” candidate for surgery. Most patients are recommended to proceed. In some cases, additional education, support, treatment, or a temporary delay may be recommended to help optimize long-term outcomes.

• Weight and medical history
• Previous weight-loss efforts
• Eating behaviors and lifestyle habits
• Mental health history
• Substance use history
• Social support
• Understanding of surgery and postoperative expectations
• Goals and motivations for surgery

The Three-Appointment Evaluation Process

1. Diagnostic Assessment

During the first appointment, we review your medical, psychological, and social history and discuss your goals and expectations for bariatric surgery.

2. Psychological Assessment

During the second appointment, you complete standardized questionnaires and screening measures related to emotional functioning, coping strategies, eating behaviors, and other factors relevant to surgical preparation and long-term success.

3. Feedback Session

During the third appointment, we review the results together, discuss individualized recommendations, answer your questions, and identify next steps.

Each appointment lasts approximately one hour and can be completed by telehealth. If your bariatric program has required forms or specific evaluation guidelines, please provide them at the beginning of the process.

What Happens After the Evaluation?

You will receive a written psychological evaluation report, a feedback session, and individualized recommendations. With your authorization, the report will be provided to your bariatric treatment team. The final decision regarding surgery is made by your medical and surgical providers.

Insurance Information

ThriveWell Psychology participates with several insurance plans, including Aetna, Blue Cross Blue Shield, HealthPartners, Medica, UnitedHealthcare, UMR, and others. Because coverage varies by plan, contact your insurance carrier to verify:

• Whether ThriveWell Psychology is in network
• Whether prior authorization is required
• Your deductible, copayment, and coinsurance
• Your expected out-of-pocket costs

ThriveWell Psychology will verify prior authorization requirements and assist with authorization when applicable. Patients remain responsible for understanding their insurance benefits and financial responsibility.
